USPS Form 1583 Interactive Checklist
Use this checklist to get organized before activating a U.S. commercial mailbox. It follows the current USPS form requirements, including identity documents and signature witnessing.
This checklist is informational and does not replace your mailbox provider's verification workflow or the official USPS form.

Current USPS ID rules
Two ID types are required for the applicant and any authorized individual: one government-issued photo ID and one address ID that confirms the address shown on the form. A driver's/nondriver's ID may be used for either role, but not both.

- U.S. state/territory/tribal driver's or nondriver's ID
- Uniformed Service ID
- Passport
- Certificate of Naturalization
- U.S. Access Card
- Matricula Consular
- U.S. Permanent Resident Card
- U.S. University ID Card
- NEXUS Card

- U.S. state/territory/tribal driver's or nondriver's ID
- Current lease
- Mortgage or deed of trust
- Home or vehicle insurance policy
- Vehicle registration card
- Voter card
The address document must contain the address entered on the form.
Signature witnessing — important
You do not always need a notary. USPS says the applicant must sign or confirm the signature in the physical or virtual, real-time audio/video presence of the CMRA owner, manager, or authorized employee; alternatively, the applicant may acknowledge the signature in the physical or virtual presence of a notary public commissioned in a U.S. state, territory, possession, or the District of Columbia.
Do not sign early if your provider's workflow tells you to wait. Follow the provider's live-verification or notarization instructions.
